To transfer services into your name please fill out the Sewer and Garbage Agreement Form completely. Emergency contact needs to be someone who lives outside the home. Also include the settlement disclosure page from youur closing documents for proof of ownership. Drop off to city offices or email to utility@hoopercity.gov. Set up will not be completed until all required documents have been submitted.
